I understand that Champions Summer Camp is full of competitive, fun games and I acknowledge that any activity contains inherent risks of injury and damage to my child and/or their equipment. Knowing these facts I nevertheless, in consideration of my child being able to participate in activities, hereby, for myself, my heirs, executors and administrators, waive, release and discharge and hold harmless: The International Church of the Foursquare Gospel, the Central District of Foursquare Churches, Midland Foursquare Church (DBA Expansion Church) and all other persons and organizations in any way connected with the events, property, and lessons associated with a youth activity that my child may engage in.  

In the event that my child needs medical attention during Champions Summer Camp, I give permission for camp staff to administer over the counter medication(s) and/or first aid as necessary and/or seek emergency medical treatment.

Please discuss this with camper or leader and acknowledged you and they agree. 

-Leave phones and devices at home. No phones or devices will be available to the camper during the camp. The camp is not liable for lost, stolen, or broken devices. We recommend leaving them at home. 

-Bring a lunch every day. No nuts of any kind nor peanut butter in lunches or snacks. 

-Ensure plans are made to arrive on time and pick up camper(s) on time. 

-Dress casual for outdoor and indoor activities. We recommend putting sunscreen on kids. 

-We expect all campers participate in activities as they are able, and show respect and kindness to leaders and other campers. Verbal or physical altercations will not be tolerated. If a camper does not act with respect or kindness, the camp coordinator will first give a warning and have a conversation with the camper; if the problem persists the parent/guardian will be contacted; If the problem still persists we may exercise the option of not permitting the camper to continue with the camp (though this is not our desire to do so). 

-We will take pictures and video of campers and use for future promotion.
